For providers

Two easy ways to arrange specimen collection for your patients

Refer your patient to book and pay for their collection directly, or coordinate a provider-arranged collection with our team.

Send laboratory orders by secure fax to (971) 200-8669, or give the order to your patient to upload during online booking. Contact orders@stumptownlabservices.com for email instructions before sending any patient information.

Referral pathways

Choose how the collection will be arranged and paid for

Stumptown's collection fee is separate from diagnostic laboratory testing charges, which are billed by the performing laboratory.

Pathway 1 · Most common

Patient-paid referrals

  1. 1Your office gives the patient the laboratory requisition, or faxes it to (971) 200-8669.
  2. 2Your patient books online at stumptownlabservices.com/book.
  3. 3Your patient pays Stumptown's collection fee during booking.
  4. 4Stumptown performs the collection and coordinates delivery to the designated laboratory.

Pathway 2

Provider-arranged collections

  1. 1Your practice contacts provider coordination to arrange the collection.
  2. 2Provider-paid billing requires the applicable executed agreement and an active service schedule. Submitting an order or an application does not activate billing.
  3. 3Scheduling and billing follow your contracted workflow.
  4. 4Specimens go to the ordered laboratory, which performs testing and reports results.

What Stumptown provides

Eligible specimen collection, preparation when required, delivery or shipping, and any specifically agreed on-site service. We do not perform every laboratory test; the designated laboratory performs outside testing and reports results to the ordering provider.

Clinical responsibilities

The ordering provider selects the tests, supplies clinical instructions, interprets results, and directs treatment, including medication or dosing decisions. Stumptown does not replace the patient's healthcare provider. Check our service area.
