How your price is calculated
Service fees are per patient and per service. Your first 25 round-trip miles and first 30 minutes of travel are included, so a travel charge applies only beyond that. When it does, it is applied once for the appointment no matter how many patients or services it covers, and appointment-time charges never stack. You'll see your complete Stumptown price before payment.

These are Stumptown's service prices. Additional travel or appointment-time charges apply only when your visit exceeds the included allowances or you choose an eligible premium appointment time. Laboratory testing charges may be separate unless specifically included in the service.

Scheduling blood draws for more than one person at the same address? When collections take place during the same appointment, each additional eligible patient pays a reduced collection fee.

Serum-tear prices include your blood draw, preparation of your prescribed serum tears, and return delivery. SneakPeek laboratory testing and PT/INR PLUS finger-stick testing are included in their listed prices. Venous blood draws sent to an outside laboratory may have separate laboratory charges.
Program pricing
PT/INR PLUS
For patients on ongoing anticoagulation monitoring who need more than one visit in the same calendar month. This pricing is separate from ordinary collection pricing.

The $39 rate applies when the same patient has an earlier completed, eligible PT/INR PLUS visit in the same calendar month, based on Pacific time. An upcoming or incomplete appointment does not qualify. Both finger-stick and blood-draw visits count. Applicable travel and appointment-time charges apply to each visit.
About PT/INR PLUS →Travel charges
We calculate a round-trip driving route from 1355 NW Everett St, Suite 100, Portland, OR 97209. The first 25 round-trip miles and 30 minutes of travel are included. Additional mileage costs $0.79 per mile, and additional travel time costs $0.60 per minute. These amounts are combined into one travel fee for the visit. The mileage and time allowances are independent. Routes at or beyond 100 miles or 120 minutes are quoted individually.

Appointment times and additional charges
Every time below is an appointment start time. An appointment-time charge is added only when your chosen start time calls for one, and only the highest applicable charge applies — these charges never stack. Available times depend on the service you select. Booking shows the available appointments and applicable charges.

Understanding your costs
Stumptown service fee
Your Stumptown fee covers the collection service provided by Stumptown Mobile Lab Services. Payment is made directly to Stumptown, and we do not bill your health insurance for this fee.
Laboratory charges
Outside-laboratory testing may be billed separately, depending on your service. SneakPeek laboratory testing and PT/INR PLUS finger-stick testing are included in their listed service prices. When applicable, you may provide insurance information for inclusion with your laboratory documentation. Coverage, deductibles, copayments, coinsurance, and other patient responsibility are determined by the laboratory and your health plan.
Need a superbill?
Stumptown can provide a superbill for our collection service upon request. Reimbursement is determined by your health plan and is not guaranteed.
If your test kit does not include prepaid postage, postage costs extra. We pay the shipping cost upfront and send you an invoice you can pay online.

Policies
Cancellation & rescheduling
The rules that affect what you pay if plans change.
- 24 hours or moreAt least 24 hours' notice is required to cancel or reschedule without charge.
- Less than 24 hoursLess than 24 hours' notice is a late cancellation, and fees already incurred may be non-refundable.
- After dispatchOnce a phlebotomist is dispatched — has departed for your appointment — the collection fee plus any Travel Fee and appointment-time charge are earned and non-refundable for client-side reasons.
- Client-side reasons include a client cancellation after dispatch, a no-show or client unavailability, a declined collection, missing identification, a missing required order or paperwork, an unavailable participant, or denied access.
- Stumptown may stop or cancel an appointment if the phlebotomist reasonably believes the environment is unsafe, unsanitary, or unsuitable. If this happens after dispatch, the appointment is non-refundable.
- If Stumptown causes the paid service not to be delivered — for example, the phlebotomist arrives without necessary equipment — you may choose a replacement visit at no extra charge or a refund. An unsuccessful draw by itself is not automatically a Stumptown error.
- If Stumptown cancels for weather, staff illness, an emergency, or another issue within our control, you get a full refund or a rescheduled visit at no charge, your choice.
- Stumptown may, in its sole discretion, waive or reduce an otherwise applicable cancellation, rescheduling, or late fee in compassionate circumstances, including acute medical emergency, patient hospitalization, death in the immediate family, or another documented emergency.
