Terms of Service

These terms apply whenever you book, pay for, or receive a mobile specimen collection from Stumptown Mobile Lab Services.

Version 2.0 · Last updated October 16, 2026

1. Who we are and what we do

Stumptown Mobile Lab Services is a mobile phlebotomy and specimen-collection company based in Portland, Oregon. We travel to a patient's home, workplace, or care facility, collect the specimen a licensed provider has ordered, and deliver it to the laboratory or carrier designated for that order.

Except for the limited point-of-care tests described below, we do not perform laboratory testing, and we do not interpret results or diagnose conditions. For most services, testing is performed by the outside laboratory named on your order or associated with your test kit, and results are released by that laboratory to your ordering provider (or, for direct-to-consumer kits, to you) under their own policies and timelines. Questions about a result belong with the provider who ordered it.

Stumptown holds a CLIA Certificate of Waiver (38D2313450) and performs a small number of CLIA-waived point-of-care tests itself: PT/INR fingerstick testing, whose device value we report to your ordering provider or anticoagulation clinic, and non-DOT rapid urine drug screening, which produces a presumptive screening result that may require laboratory confirmation. We do not interpret these results or direct your care.

Nothing on this site and nothing said by our staff is medical advice, a diagnosis, or a treatment recommendation. We do not decide which tests you need.

2. Booking and eligibility

You must be 18 or older to create a booking. A parent, legal guardian, or authorized legal representative may book and provide consent on behalf of a minor or an adult who cannot consent for themselves. A representative may provide consent electronically during booking or through a secure completion link; our team verifies their authority before a phlebotomist is dispatched. A representative who has already completed that electronic consent and whose authority has been verified does not need to be present solely to sign again. A minor must still be accompanied by a parent, guardian, or other responsible adult during the visit, and we may require a representative to be present when the patient's safety, supervision, or identity confirmation requires it.

Most services require a valid order, requisition, or prescription from a licensed provider. Services that a customer may order directly are marked as such during booking. If required paperwork is missing, incomplete, or expired when we arrive, we may be unable to collect and cancellation terms in section 5 apply.

You are responsible for the accuracy of the information you provide, including the patient's identity, the service address, and how to reach you. We verify patient identity at the appointment and may decline to collect if identity cannot be confirmed.

3. Pricing and what our fee covers

For most services our fee covers travel and professional specimen collection. Quoted prices may include a base service charge plus travel-distance and appointment-timing components, all shown to you before you pay.

Some services include more than collection in the price shown: the point-of-care tests we perform ourselves (PT/INR fingerstick testing and non-DOT rapid urine drug screening), SneakPeek Clinical collection and testing as described on its service page, and autologous serum-tears preparation and same-day return. The service page and your booking summary state what each price includes.

Except where the selected service expressly includes testing in its price (as described above and on the service page), laboratory testing referred to an outside laboratory is billed separately by that laboratory, not by us. Any charge from the laboratory, your provider, or your insurer is between you and that party. We do not control those amounts and cannot quote them.

For test-kit collections with processing and shipment, non-prepaid postage, courier and carrier charges are not included in the price you pay at booking; when a kit's postage is not prepaid, those charges are invoiced separately afterward. Your complete Stumptown service total is always shown before payment.

Prices shown on this site may change. The price that applies to your appointment is the price confirmed at the time of payment. If a booking sits unpaid long enough for pricing to change, we re-price it before checkout and show you the updated total.

4. Payment

Payment is collected at the time of booking through our payment processor. We do not store full card numbers. A confirmed appointment requires completed payment unless we have agreed otherwise in writing, such as under an employer or research agreement.

5. Cancellations, rescheduling, and no-shows

You may cancel or reschedule at no charge with at least 24 hours’ notice before the start of your scheduled appointment time. With less than 24 hours’ notice, the cancellation is late and fees already incurred may be non-refundable. Once a phlebotomist has been dispatched — meaning the phlebotomist has departed for the appointment — the collection fee plus any travel and premium-time charges are earned and non-refundable for client-side reasons: a cancellation after dispatch, a no-show, missing identification, a missing required order or paperwork, denied access, or unsafe conditions at the location.

If nobody is present at the address at the scheduled appointment time, if access to the patient is denied or not possible, or if the patient declines collection after we arrive, the visit is treated as a no-show and the full service fee applies. If the phlebotomist stops a dispatched visit because they reasonably believe conditions are unsafe, unsanitary, or unsuitable, the charges are likewise non-refundable.

If our phlebotomist cannot complete the collection for a clinical reason — difficult venous access, a patient who is unwell, or a safety concern — we will tell you what happened and work with you on the next step. An unsuccessful draw by itself is not automatically a Stumptown error. We do not charge for a second attempt made necessary by our own equipment or process failure.

We may cancel or reschedule for severe weather, road closures, illness, or staffing emergencies. In those cases you get a full refund or a rescheduled visit, your choice.

Stumptown may, in its sole discretion, waive or reduce an otherwise applicable cancellation, rescheduling, or late fee in compassionate circumstances, including acute medical emergency, patient hospitalization, death in the immediate family, or another documented emergency.

6. Refunds

We refund in full when we cancel. If Stumptown causes the paid service not to be delivered — for example, the phlebotomist arrives without necessary equipment, or we cannot reach the location for a reason within our control — you may choose a replacement visit at no extra charge or a refund. This does not apply to the client-side reasons and unsafe conditions described in section 5, and an unsuccessful draw by itself is not automatically a Stumptown error. We do not refund a completed collection based on the laboratory's result, turnaround time, or billing, because those are outside our control. To raise a refund question, call (971) 204-6819.

7. Your responsibilities at the appointment

Please provide a safe, reasonably clean, well-lit space with somewhere for the patient to sit or lie down, and secure any animals during the visit. Tell us in advance about anything that affects safe access, such as stairs, gate codes, or parking restrictions.

Follow any preparation instructions on your order. Preparation is service-specific; fasting is not universally required and we will never invent an instruction your provider did not give.

8. Safety and our right to decline

Our staff may end or decline a visit when a situation is unsafe, when someone is threatening or abusive, when the patient's condition makes collection clinically inadvisable, or when the environment does not allow a sanitary collection. Safety of patients and staff comes before completing a visit.

9. Workplace and employer services

All workplace drug-testing services we provide are non-DOT only. We do not perform DOT-regulated testing of any kind. Employers using our services are responsible for their own testing policy, notice, and compliance with applicable law.

10. Health information

How we handle health information is described in our Notice of Privacy Practices, and how this website handles ordinary web data is described in our Privacy Practices. Please do not send test results, diagnoses, or clinical history through website forms or ordinary email.

11. Text messages (SMS)

By opting in, you agree to receive text messages from Stumptown Mobile Lab Services related to appointment scheduling, confirmations, reminders, service updates, and customer support. Message frequency may vary. Message and data rates may apply. Reply STOP to opt out or HELP for assistance. Consent is not a condition of purchase or service.

Stumptown does not send promotional or marketing text messages. Opting in to text messages is always optional and is separate from accepting these Terms of Service.

12. Website use

You may use this site to learn about and book our services. Do not attempt to access accounts, portals, or records that are not yours, scrape or overload the site, or interfere with its operation. Our name, logo, and site content belong to us.

Portal accounts for providers, staff, and employer partners are for named, authorized individuals. Keep your credentials confidential and tell us immediately if you believe an account has been compromised.

13. Limitation of liability

To the fullest extent permitted by Oregon law, our total liability arising out of a given appointment is limited to the amount you paid us for that appointment. We are not liable for indirect or consequential losses, for the acts or results of a laboratory or provider, or for delays caused by circumstances beyond our reasonable control.

Nothing in these terms limits liability that cannot be limited by law, including liability for our own negligence causing personal injury.

14. Changes to these terms

We may update these terms. The version in effect for your appointment is the version published on the date you booked. Material changes will be reflected in the "last updated" date above.

15. Governing law and contact
